If you have a carer and would like your carer to be able to collect any test results or deal with any aspect of your healthcare please speak to a receptionist or fill in our Register as a Carer form. Once you and your carer have given permission on the carer form we will update your records accordingly.
If you are a Carer (this does not include looking after children who do not have extra needs), we can then invite you to relevant health promotion appointments in the Surgery and to Carers events held locally.
It is important that we know if you are a carer so that we can make sure you receive information, services and the help that is available.
Also we have a Carers notice board in Reception that has information you might find useful.
Remember, a carer is anyone who cares, unpaid, for a friend or family member who due to illness, disability, a mental health problem or an addiction cannot cope without their support. Find out more on the Carers in Hertfordshire website.
At the surgery we have a team of Care Co-ordinators who can help carers with all aspects of being a carer. This includes taking care of yourself as well as the person(s) you are caring for. A member of the surgery team may signpost you to the Care Co-ordinator team or you can ask reception for them to contact you.
